I, the undersigned, hereby agree to consign the item (s) listed to Consign of the Times Boutique, 800 Turnpike Rd., New Ipswich, NH 03071 for a period of 60 days. Prices will be set by management at the time of consignment. Sales are determined by management. No pick-ups will be allowed before 60 days.

Our accounting systems are fully automated, and your items are tagged, scanned and link into our computer with your very own consigner I.D. number and are ready for immediate sale within 24 hours of receipt.

If the items are sold within the 60-day period, I will receive 40% of the purchase price for clothing and other items. After 30 days, merchandise is automatically reduced to 20% for the item. Out-of-season and overstock items are subject to clearance sales. COTTB reserves the right to mark down any items  at any time and make price adjustments according to market demand.  COTTB  has the discretion to donate damaged , or stained items dropped off for consignment upon 2^nd look without prior notice to the consignor.

After 60 days my items will expire, and will become property of Consign of the Times Boutique. COTTB will donate expired items to local charities at their discretion. Should you desire to pick up your items, you must pick them up by the pick-up date listed on your Item List. Any items left past the pick-up date may donated to local charities to make room for new consignment. We cannot hold you items aside to due space constraints. _______ (Please initial)

My check will be ready for pick-up in the store by the 5th of each month for amounts of $10.00 or more occurring in the previous month. Any amount below $10.00 will accrue until the following month's check.  Checks not picked up by the 5th of the next month may be mailed to me upon request, for a fee of $2.00. This amount will be deducted from my account balance. Checks are only valid for 90 days and must be cashed within this time frame or money will be re-deposited into the store account.  I will not be compensated on any expired items.

I hereby warrant that I have completed title to the merchandise listed, and I will not hold Consign of the Times Boutique responsible for any claim or title to it. I also understand that Consign of the Times Boutique assumes no responsibility for loss or damage to listed merchandise by fire, theft, accident or any other cause.
